Adverse events include a side effect, reaction or other problem with a medicine, vaccine or medical device. Your report can help keep others safe.

Report a medicine or vaccine adverse event

Call 1300 633 424 Monday to Friday, 9am to 5pm AEST. A registered pharmacist will give advice about your issue with a medicine or vaccine. This service is free to Australians.

Make a report through the Adverse Event Management System. Give as much information as you can.
Report a medical device incident

Make a report through the Medical Device Incident Report system. Give as much information as you can.

Other reports
You can make reports for other issues related to medicines, vaccines and devices.
This includes suspected illegal importation, manufacture, supply, commercial possession or advertising of vapes and vaping goods.
