Report a medicine or vaccine adverse event

You can report a medicine or vaccine adverse event to us online directly using the link in the box below.

You can also contact a health professional - your GP, nurse practitioner, midwife or community pharmacist - to help you report an adverse event.

    Make a report through the Adverse Event Management System. Give as much information as you can. 

Report a medical device incident

    Make a report through the Medical Device Incident Report system. Give as much information as you can.

Report a medicine or vaccine defect

Other reports

You can make reports for other issues related to medicines, vaccines and devices. 

This includes suspected illegal importation, manufacture, supply, commercial possession or advertising of vapes and vaping goods.
