Report a problem

Understand why reporting issues with medicines and medical devices is important, and how to do it.

What you can report

You can report issues with therapeutic goods such as:

  • Adverse events (side effects and problems): Unintended and sometimes harmful problems linked to medicines, vaccines, or medical devices.
  • Product defects: Problems with how the product is made, stored or handled, such as faulty packaging or device faults.
  • Counterfeit products: Suspected fake medicines or medical devices.
  • Non-compliant advertising: Promotional materials that don't meet advertising standards for therapeutic goods.
  • Questionable practices or breaches: Activities that may break the law, such as selling unapproved products or importing goods without permission.

Report a problem with a therapeutic good

How you report a problem with a therapeutic good, and how we manage it, depends on the type of product involved. It also depends on whether you are a consumer, health professional or sponsor. We review every report and investigate when action is needed.
