Submitting your form

Submit the completed request accompanied by evidence of payment, by email to accountsrec@health.gov.au. 

The subject field of your email must quote your TGA Organisation Identification Number (e.g. TGA00xxxxx’) followed by ‘ARTG Reinstatement Request’.
