Use this form to provide organisation details and authority arrangements required to make applications under the Therapeutic Goods Act 1989.

This form is used to collect and maintain details about an organisation that makes applications to the TGA under the Therapeutic Goods Act 1989, including authority arrangements for individuals or third parties acting on the organisation’s behalf.

You must complete this form if your organisation:

  • supplies or manufactures therapeutic goods in Australia
  • conducts clinical trials in Australia
  • supplies proprietary ingredients
  • provides regulatory or agent services on behalf of another organisation

Before completing this form

Before submitting this form, ensure you:

  • understand your organisation’s role (sponsor, manufacturer, agent, or combination)
  • have authority to provide information on behalf of the legal entity
  • have reviewed the TGA Business Services (TBS) Terms and Conditions

This form must be completed once per legal entity. Updates are required when organisation details or authority arrangements change.

Submitting this form

Submit the completed form through the nominated channel listed on the TGA website or via TGA Business Services, as applicable.
