Certificates of Free Sale and Export Certificates can only be provided for medical devices with a current inclusion in the ARTG. Do not use this application form to apply for export certification for listed medicines.

Submitting your form

Email your completed application form, accompanying schedule (if required) and your completed credit card authorisation form to accountsrec@health.gov.au.
