Low value turnover application forms

Application for an exemption from payment of annual charges due to low value turnover for existing or new products on the ARTG

The forms below allow sponsors to apply for an exemption from payment of annual charges on the basis that the turnover for an existing or new product is less than the applicable threshold.

The forms below are for the purposes of Regulation 43AAC of the Therapeutic Goods Regulations 1990.

The pdf form can be filled in and saved to your computer using Adobe Reader version 7 or later or any version of Adobe Acrobat Standard or Professional. If you have an earlier version of Adobe Reader you can fill in the form on-screen and print it out but you will not be able to save the completed form.
