Problems with submitting some online forms on the TGA website (www.tga.gov.au)
We did not receive some online forms submitted between 29 October and 12 November 2014
Due to a problem outside of the TGA's control and which has only just come to our attention, we did not receive some online forms submitted between 29 October and 12 November. The only forms affected were:
- Consumer report a side effect to a medicine or vaccine. (Note this form was fixed on 5 November - only resubmit forms sent between 29 October and 4 November.)
- Report a medicine deficiency or defect
- Register as a small business with the TGA
- Website feedback or error reporting
We also experienced issues with requests to subscribe to all email updates.
Please resubmit your online form and it will be actioned as soon as possible. We apologise for the inconvenience.
This issue was a result of additional security measures on our new servers and has now been resolved.
