Updating organisation administrator
- Use this form to nominate a new representative from your organisation to be the administrator. This person will be able to add, remove and update contact details.
- Declaration: Please ensure a signed form is sent to the Therapeutic Goods Administration
- Once you submit a completed form the TBS Helpdesk will email the new administrator within three working days with the unique user name and administrator guide. This will be sent to the email address provided on the submitted form.

This form is available in pdf and Microsoft Word formats. The pdf form can be filled in and saved to your computer using Adobe Reader version 7 or later or any version of Adobe Acrobat Standard or Professional. If you have an earlier version of Adobe Reader you can fill in the form on-screen and print it out but you will not be able to save the completed form.
