How to report an adverse reaction
Adverse reactions to medicines and vaccines can be reported:
Report using the 'Blue Card'
You can download and complete the 'Blue Card' reporting form. Blue Cards are also available from the TGA Office of Product Review.
Send completed Blue Cards to the TGA:
- by mail to Office of Product Review, TGA, Reply Paid 100, Woden ACT 2606
- by fax to 02 6232 8392
- by email to adr.reports@tga.gov.au
Report by phone
Consumers can call the Adverse Medicine Events Line on 1300 134 237.
The Adverse Medicine Events Line is open from Monday to Friday from 9am to 5pm AEST.
This phone-in service, provided by the Mater Hospital in Brisbane, is available for members of the public who suspect they have experienced an adverse medicine event. The service forwards reports of suspected adverse reactions to the TGA Office of Product Review.
