Supply of information for the creation of a Provisional ARTG Record (PAR)

Sponsors can apply (using this form) to the Drug Safety and Evaluation Branch (DSEB) Application Entry Team (AET), for the creation of a Provisional ARTG record. If the product is granted approval, the Provisional ARTG number will become the AUST R number, which is to be included on product packaging.
Sponsors should note that although this form allows electronic completion, the completed application form must still be submitted in hardcopy, and included as Module 1.12.
