Report of suspected adverse reaction to medicines/vaccines

Reports of adverse reactions to medicines/vaccines can be made any of the following ways.

Report by phone

Call the consumer Adverse Medicine Events Line: Ph 1300 134 237

This phone-in service, provided by the Mater Hospital, Brisbane, is available for members of the general public who suspect they have experienced an adverse medicine event. The service forwards reports of suspected adverse reactions to the Adverse Drug Reactions Advisory Committee.

Report using form ("Blue Card")

How to access a pdf document

This form can be filled in and saved to your computer using Adobe Reader version 7 or later or any version of Adobe Acrobat Standard or Professional. If you have an earlier version of Adobe Reader you can fill in the form on-screen and print it out but you will not be able to save the completed form.
