The following forms are for the use of sponsors or manufacturers mainly regarding the payment of various TGA fees and charges.

Authorisation to cancel a licence from the ARTG
Use this form to request cancellation of medicines or devices from the Australian Register of Therapeutic Goods
Client details form
This form should be completed by or for each client involved with the supply of therapeutic goods in Australia for which the TGA requires address details
Credit card payment authorisation
Credit card payments can be made for TGA invoices, applications, publications and seminars
Electronic funds transfer agreement
Electronic funds transfers will be accepted following completion of this form
Electronic invoices nomination form
This form allows you to nominate to receive electronic invoices from the TGA
Low value turnover application forms
The forms allow sponsors to apply for an exemption from payment of annual charges on the basis that the turnover for an existing or new product is less than the applicable threshold
Publication order form
This form is to order a TGA publication that requires payment
Register as a small business with the TGA
The TGA is asking all of its vendors to identify whether they should be considered as a small business by the TGA in making payments
Request to transfer the sponsorship of an ARTG entry
Use by the accepting company and the relinquishing company to request to transfer the sponsorship of an ARTG entry
